By engaging in coaching services with Zensations Health & Wellness Inc., you agree to the following:

I understand that I am participating in services provided as a Life Coach and Holistic Health Coach.

These services are designed to support:

• Personal development
• Lifestyle improvement
• Goal setting and accountability
• General wellness and self-awareness

I acknowledge that:

• Coaching services are not medical, psychological, or regulated healthcare services
• The coach is not a licensed physician, psychologist, therapist, or dietitian
• Coaching is not a substitute for professional medical or mental health care

I agree to seek appropriate licensed professionals for medical or psychological concerns.

I understand that:

• I am fully responsible for my own decisions, actions, and results
• All choices made during and after coaching sessions are my own
• Results are not guaranteed

All personal information shared during sessions will be kept confidential, except:

• Where disclosure is required by law
• Where there is risk of harm to myself or others

I confirm that:

• I am participating voluntarily
• I may stop coaching at any time
• I am 18 years of age or older

• Payment is required prior to or at the time of service
• Cancellation policies are outlined separately
• Missed appointments may be subject to fees

To the fullest extent permitted by law, I agree that Zensations Health & Wellness Inc. is not liable for:

• Decisions I make
• Actions I take
• Outcomes resulting from coaching services

By booking or participating in services, I acknowledge that I have read, understood, and agree to this Client Consent & Coaching Agreement.
